At 44 km altitude, while stage 1 was still burning, stage 2 experienced an overpressure and disintegrated. Stage 1 continued to fire for a few seconds but was destroyed or shut down, and debris fell to the ocean. The accident was blamed on mechanical failure of a strut supporting a helium pressure vessel.
